White (my great-grandmother, your great-great-grandmother) filed in order to receive a pension from the State for being the surviving widow of A. B. (Amonett Berkeley) White. Note that the statement written sideways along the left margin ("Dr. C. Dickinson, who attended Ambrose B. White in his last illness is dead") has his first name wrong. The printed form is legible; to help you fill in the blanks for the hand-written parts are the following: Ann B. White ........Pittsylvania 28th day of April, 1900 · I am 68 years of age · Died at home near Brow, Va (Febry?) 13, 1885 · death resulted from having been exposed during service in war (where?) he contracted Consumption (now - Tuberculosis) · Certified by the Deputy Clerk, O.S.B. Yeatts · Witnessed by W.H. Haley (also certified by the Clerk) · Certified by Commr.of Revenue, F.O.Oakes (Note that no value is given for the Estate) · Certified by Physician, Dr. R.T.Ramsey, who states Col. A. B. White's death resulted from Consumption & disease of the heart · Certified the Clerk, S.S. Hunt to receive $25.00 annually Note, also, this refers to an original application filed in April, 1900 - however this is certified in 1902.
